TMIT Wants Your Stories:

 

TMIT documentaries such as Chasing Zero: Winning the War on Healthcare Harm and Surfing the Healthcare Tsunami: Bring Your Best Board (as seen on the Discovery Channel) focus on healthcare accidents and how to fix the systems failures that cause them, and the roles that governance leaders from communities and citizens play to drive action.

If you have a compelling story about an adverse event that happened to you or a member of your family, a hero story about a CareDad or CareMom who improved care in his or her community, or a story about the heroic behavior of a caregiver that you would like to share, please complete the form below.

At this time, you only need to write a short story summary. Let us know briefly:

  1. Who are the characters in the story? (You do not need to use their names.)
  2. What happened? What was the adverse or heroic event?
  3. What were the behaviors that saved patients from harm, or that caused harm?
  4. What was the final outcome?
  5. What was the lesson learned from the story?

If we choose to highlight your story in a future movie, we will contact you for further information and more detail. In order to maintain confidentiality, please do not use the names of care facilities or caregivers.

We will not be able to use everyone’s story, because many of the stories will be similar. We plan to highlight a number of stories that are compelling and representative of healthcare accidents with lessons to be learned, and of the heroic behaviors of the great people who prevent harm: consumers and caregivers.
